Comprehensive Foot Assessments

Gentle Foot and Wound Care provides comprehensive mobile foot assessments for older adults in their homes and senior living communities. Each assessment considers the patient’s skin, toenails, sensation, comfort, mobility, and individual foot-care needs. Findings are explained in clear, patient-friendly language, with personalized recommendations and referrals when appropriate.

What the Assessment May Include

A comprehensive foot assessment provides a closer look at changes that may affect comfort, safety, and mobility. The assessment is personalized according to the patient’s health history and individual foot-care needs.

The assessment may include:

• Observation of the skin, toenails, and spaces between the toes
• Identification of corns, calluses, pressure areas, or skin changes
• Monofilament screening for reduced protective sensation when appropriate
• Observation for swelling and changes in skin color or temperature
• Review of foot discomfort, mobility, and footwear concerns
• Identification of wounds or other areas requiring attention
• Evaluation of the patient’s ability to safely perform foot care
• Recommendations for appropriate follow-up or additional evaluation

Who May Benefit from a Foot Assessment?

Mobile foot assessments may be helpful for older adults who have difficulty safely examining or caring for their feet. Regular professional observation may also help identify changes that should be monitored or discussed with another healthcare provider.

An assessment may benefit people who:

• Have diabetes, reduced sensation, or circulation concerns
• Have difficulty bending, reaching, or seeing their feet
• Experience recurring foot discomfort or pressure areas
• Have thick or difficult-to-manage toenails
• Notice changes in their skin, toenails, or foot shape
• Have limited mobility, balance, vision, or hand dexterity
• Live at home, in assisted living, or in a senior living community

Personalized Recommendations

After the assessment, findings are reviewed with the patient in clear, understandable language. With the patient’s permission, family members or caregivers may also participate in the discussion and learn how to support safer foot care between visits.

Recommendations may include:

• Professional nail or skin care
• Guidance for safer foot care between visits
• Strategies for reducing pressure or friction
• Footwear considerations to support comfort and safety
• Changes that should be monitored or reported
• An appropriate schedule for follow-up visits
• Referral to another healthcare provider or specialist when needed

When Additional Medical Care May Be Recommended

Some findings require diagnostic testing, specialized treatment, or medical care beyond the scope of a mobile foot-care visit. When this occurs, we explain what was observed, why additional evaluation may be helpful, and what type of healthcare provider or specialist may be appropriate.

Additional evaluation may be recommended for:

• Open, deep, or slow-healing wounds
• Increasing redness, warmth, swelling, pain, or drainage
• Significant numbness or loss of protective sensation
• Circulation concerns or changes in skin color or temperature
• Persistent or worsening foot pain
• Structural changes affecting comfort, balance, or mobility
• Conditions requiring diagnostic testing, imaging, or advanced treatment
